This is caused by a duplication in the AV Database

1. Have customer show you the Modifier that has the issue
2. Look in Modifier list to see if they already have a modifier with a duplicate Modifier Code (e.g. “Better Image” is showing twice in the Modifier list) - delete one of the duplicates
